Who trains Maximum and how does he end up becoming a Mirage Knight and who makes him a kensi and what mortar headds does he use?

Training: unclear.
Probably Mel Rince, since not many would be capable of training him.
updateHe grew up in the courts of Tenjou.
Rognar taught him.

Mirage Knight: unclear.
But Maximum is the 12th Duke of Fates, which belongs to the AKD. (Homeland of the Ballance family)
So he is a subject of the AKD to begin with.
Maybe he got drafted by Tenjou...

Kensei: unclear.
It seems from a small insert (which I think got edited out in final publication) that Madora Moirai makes him Kensei.
Madora may be qualified to appoint a Kensei because she is (sort of) a Kensei herself. (Les Hyper Chevelier du Rose)

Mortar Headd: Mighty Alpha, aka LED Mirage B4 Destonias, aka Akatsukihime
He also piloted a 'regular' LED Mirage no. XXIII

Falk Rognar for an instructor? Damn! That's a fast track to mastery of combat. How many headdliners would pay big bucks for that kind of arrangement?

Yeah. Being the strongest Headdliner in History makes it difficult to find instructors. Max would quickly surpass anyone who could teach him. Since Kaien is dead the only one's who could teach him anything worthwhile would be Ena Dai Gu, Madora Morai, Mel Rince and of course, Rognar. Other than that, he might be able to learn a technique or two from individual knights that the meets, but he couldn't improve his skill by sparring with them, as they would be no competition whatsoever. He's stronger than everyone except Rognar, so Its pretty much a given that Rognar is the most logical sparring partner.
